FORDHAM Quarantine Confirmation Form
As you know, New York State has imposed a mandatory quarantine on individuals traveling from a state that is NOT contiguous to New York, including if outside the United States. All travelers are encouraged to quarantine for 10 days upon arrival, but you can now “test-out” of quarantine by 1) having negative test results of a FDA EUA authorized PCR saliva or nasal test administered within three days prior to your arrival; 2) quarantine for 3 days (cannot be done in University housing); 3) on day 4, obtain another COVID test either on or off campus; 4) If the pre-departure test AND this second test come back negative, you may exit quarantine upon receipt of the second negative diagnostic test.

All students with home addresses in states not contiguous to New York or with an international address (residents as well as commuters) must complete this form to gain access to campus for spring semester.

Please see more detailed instruction in the email you received on December 10, and regularly check your email for changes in these requirements as the University is closely monitoring the state's changing guidelines.

Complete this form at the end of your quarantine period and immediately prior to move-in or arrival on campus for classes, Orientation, or any other activities.
